Account Set Up Forms

To set up an account with DataTrace, complete the
appropriate forms below and email them to info@datatraceonline.com or fax them to us at 801-253-2478

Credit Report Requirements: A site survey must be conducted at your office or place of business before we can set up your account for access to employment or tenant screening credit reports. The fee to do the site survey is $100.00.  If you do not require credit reports then there is no site survey required and there is no set up fee. Credit reports are only available for employment or tenant screening purposes.
